Apple recalls iPhone 3G power adapter for shock risk
Apple on Friday 19, September 2008, announced free replacement of power adapters for iPhone users in the US, Canada, Mexico, Japan and Latin American countries due to shock risk of the adapter that comes with the 3G iPhone.

Though there are no reports about any injuries caused because of the adapter, the announcement outlines that under certain conditions the USB power adapter metal prongs can snap and remain in the power outlet leaving a live exposed metal that poses shock to users.
Here is an excerpt from the announcement:
We have received reports of detached blades involving a very small percentage of the adapters sold, but no injuries have been reported.
Users of the adapter are advised to immediately stop using the adapter and submit them to Apple Product outlets for replacement. The replacement either online or through Apple retail outlets will start from October 10, according to the report. Those who purchased the adapter as stand alone accessories are also eligible for the replacement.
